Dr. Hernandez earned her Doctor of Nursing Practice degree from the University of the Incarnate Word in San Antonio, Texas. Dr. Hernandez has 10 years of patient care experience. Using a holistic approach, Dr. Hernandez has provided a comprehensive care plan to her patients by addressing general health, weight management, mental wellness, pain, and self-image.
Also, Dr, Hernandez is well experienced in providing ketamine treatments, PRP injection and botox (for headaches).
Dr. Aditya Shah, DO
Medical Director
Dr. Shah serves as the Medical Director at Mendala PolyTrauma Clinic and is a Board-Certified Psychiatrist with specialized expertise in the nexus of traumatic brain injury and psychiatry. As a Certified Brain Injury Specialist (CBIS), Dr. Shah focuses on creating comprehensive, multi-modal treatment plans for patients with TBI and concomitant psychiatric conditions. His clinical protocols integrate cutting-edge, evidence-based modalities, including Ketamine Treatment, VR Exposure Therapy, and TMS, to target resistant pathology and optimize functional recovery.
Dr. Shah’s distinct clinical approach was cultivated during his residency at the Texas Institute for Graduate Medical Education and Research.
Beyond his clinical practice, Dr. Shah, in partnership with Dr. Rubaye, furthers the clinic’s scientific mission through active research. Together, they investigate the neurobiological mechanisms and clinical outcomes of Ketamine and TMS in TBI populations, contributing to the evolving evidence base for interventional neuropsychiatry.
Dr. Shah also serves on the Clinical Advisory Board for Iluria Health and is a Clinical Faculty Member at the University of the Incarnate Word, where he supervises and trains residents in the management of TBI and interventional psychiatry.
Kandace Gardner

Kandace is a Certified Medical Assistant and Registered Phlebotomist with over 18 years of experience across diverse healthcare settings. uyggShe is recognized for her adaptability, professionalism, and ability to thrive in fast-paced, high-pressure environments while delivering compassionate, patient-centered care.
She currently serves in our TBI clinic, performing advanced diagnostic and therapeutic procedures such as quantitative electroencephalograms (qEEGs), eye tracking assessments, ketamine therapy, assisting with platelet-rich plasma (PRP) treatments and Botox for symptom management. Kandace is committed to ensuring patient comfort and understanding while supporting a collaborative, multidisciplinary approach to treatment and therapy.
Safa Rubaye, MD, MBChB
Director of Psychiatry and Traumatic Brain Injury Department
Dr. Rubaye, is a board-certified psychiatrist with 14 years of patient care experience. Dr. Rubaye specializes in neuromodulation, including transcranial magnetic stimulation and ketamine therapy. Dr. Rubaye was the first physician in San Antonio to provide Zulresso treatment for women experiencing postpartum depression. During his residency at the University of Texas Health Science Center at San Antonio, Dr. Rubaye took a particular interest in research, working on detecting biomarkers of dementia. Dr. Rubaye has shared his discoveries through multiple presentations and published manuscripts.
Dr. Rubaye current focus is to use his experience in neuropsychiatry to provide care to patients who suffer from traumatic brain injury. Dr. Rubaye was trained at the PolyTrauma Clinic at the VA hospital to treat veterans with TBI, PTSD, and other psychiatric illness.
Dr. Rubaye serves as a faculty member at the Texas Institute for Graduate Medical Education and Research, where he educates psychiatric residents on the complexities of mental illness and traumatic brain injury.
